Extended Care
- Referral Process: Empower Youth accepts referrals from numerous sources, including medical, legal, and educational professionals. All referrals must complete the intake process prior to being place in individual or group therapy. If the client appears to be in need of a higher level of care, Empower Youth will refer the individual to the appropriate services.
- Coordination of Care: Empower Youth is willing and able to coordinate care with legal, medical, educational, and mental health professionals. The client must sign a Release of Information to the care coordinator and approve the release of any information beforehand. The release of information can be limited in regards to the nature of information released and how comfortable the client is releasing such information.
